You need something to do NAT traversal for the far end phones...
See the section on how to traverse firewalls at http://sipx-wiki.calivia.com/index.php/SipX_HOWTO_Library The best option in a released form at this point is an Ingate Siparator (www.ingate.com) with the far-end NAT traversal option, OpenSBC (www.opensipstack.org) or go beta with the 3.11.x code and test sipxBridge (although that might only be for sip trunking at this point... not sure).
