Ranga helped me with this one offline. There were a couple of issues with my
settings. First, I had sip.unlimitel.ca as the ITSP server domain name and
ITSP server address. I changed the ITSP server domain name to unlimitel.ca
and removed the ITSP server address, as suggested by Ranga. Also, the caller
ID *must* match the phone number. For voip.ms and les.net, I had this set to
my current business phone number and it worked, but Unlimitel doesn't allow
this.

Here are what my settings ending up being (the user-name and password have
been changed):

<itsp-account>
    <itsp-proxy-domain>unlimitel.ca</itsp-proxy-domain>
    <user-name>4169999999</user-name>
    <password>xxxxx</password>
    <itsp-proxy-listening-port>0</itsp-proxy-listening-port>
    <itsp-transport>UDP</itsp-transport>
    <use-global-addressing>true</use-global-addressing>
    <strip-private-headers>false</strip-private-headers>
    <default-asserted-identity>true</default-asserted-identity>
    <register-on-initialization>true</register-on-initialization>
    <registration-interval>600</registration-interval>
    <sip-keepalive-method>CR-LF</sip-keepalive-method>
    <rtp-keepalive-method>NONE</rtp-keepalive-method>
  </itsp-account>

Also, as mentioned above, the caller ID must match the phone number /
user-name.

Ranga also sent me an updated sipXbridge.jar, but I'm not sure if it's
needed or not. I think it was just a config problem and the updated jar
isn't needed.

I created a unlimitel.xml template based on this and gave it to Ranga. He
said he would pass it on to be included with sipXecs.
