That actually suggests to me that the problem is in the interface
between your ITSP and the PSTN.

One way to check this would be to set up a PC on a hub that connects
sipXbridge to your ITSP and take a wireshark trace of a call that
exhibits the problem.  Wireshark has a facility (under Analyze RTP
Stream, if I recall correctly) that will let you save the RTP stream as
an audio file - do that and then play the resulting audio.  If the
clicks are not there, they are being introduced somewhere inside your
ITSP.
