Taking the name out worked. Putting it back in broke outgoing calls again.

I can't believe my luck in finding weird new problems lately <sigh>

For example, right now, we're testing two users, one with name, one without.
The user with name cannot dial out, the user without can.

We also just tried with a first name, can't dial, with a second name only, 
can't dial out.

Here is where I found a thread talking about this which gave me the idea to 
give it a try.
