Thanks for the replies! Setting the packetization period did improve quality, 
however other issues arrived in the meantime: NAT traversal issues. 
The setup is as following: 

ITSP <== sipxecs ==> ACD agent. 

sipxecs is running for testing purposes without firewalling on a public 
routable address (no NAT). 
the ACD agent is configured as a remote worker and is behind NAT. Calls to the 
PSTN via the ITSP are working fine, and so are locall calls between remote 
workers. 
But as soon as the ACD server enters the stage, problems arise! Incoming calls 
via the ITSP are routed trough an auto attendent to the ACD queue. 
When the agent answers, no audio is heard in both directions. The auto 
attendent is working fine. 

A sip trace points out, the call gets re-invited between the ITSP and the ACD 
agent without sipx staying in the media path. Therefore, the audio issues 
arise. 
When calling without using the ACD (either from PSTN/ITSP or local) sipxecs 
stays in the media path. 
Could anyone point me in the right direction what is causing this problem?

Make sure all ends use muLaw and set 20ms packetization period on SPA.
