You need to provide some details about your ITSP as well.

In your SBC setup, is MOH enabled?

If your audio works in both directions when taken off hold, it is not a
firewall issue.

I don't think the codec "should" matter if your ITSP offers G711 as the
valid option. Neither do I think the firmware should matter though 3.2.x has
issues with SLA and recommending 3.1.3RevC might be a good option if there
is an issue with calls going directly to voicemail for remote users.

If 211 calls another user locally (polycom to polycom) and is put on hold,
does MOH work? If so, it might simply be an option at the ITSP. Please check
with the ITSP and see if they have an option to be able to allow you to
specify MOH.It sounds like they see MOH and set it to 0.0.0.0.

Note: I did not look at your pcap's, but everything you said pointed me to
believe this is an ITSP option/implementation.
