The screaming monkeys seem to be a good solution if they place outside 
calls.
That should stop that from happening :o)

If you really need to be sure they can't be reached then disable the AA. 
If that is impossible then
to prevent calls to inside you can still setup a separate sip and forward 
the calls to that server.
If for example extension 333 needs to be "lost" then make a dialrule that 
sends 333 to the "lost" server
and define 333 on "lost", preferably with domain lost.something.
