If it is going through NAT it is NOT local. You need to make sure your NAT'ing router has its sip ALG turned off as well SPI. If the traffic does not go through your firewall but lands directly on the LAN (internally nat'd network) I don't know that there is a good use case for that since I don't recall that scenario being in any of the design possibilities "as we know the product now". The indication you get that the registration has a "privcontact" address means it is going through nat. the fact that your media is going to the interface of the router is "correct", but the router has a sip alg or spi that is preventing them from getting to the phone.
