Forward and record new message or just plain forward? Have you turned on debug logging in sipXivr and looked for bugs in logs? Are you using a polycom 301 with no microphone. You laugh, but I just did that a few weeks ago. Was a long time since i fell for that one. You seem to have the most recent or at least recent enough.
