If you're ISP is the same as your ITSP, they will usually handle the prioritization of the packets as they don't leave their network.
QoS does not follow the packet once it gets to the ISP. Unless you have a contract with them that states otherwise, ISP's are best effort services. That was one of the problems that we had with Time Warner Business Class ISP. Even though they would put voice traffic higher than regular data (even when we were using a different ITSP), as far as their QoS goes, it was still put below their own VoIP services. Which makes sense, they want their own voice traffic to be at the highest priority. Next best is a private line / mpls connection directly to your itsp where they would honor dscp settings.
