Christiaan, Adam -- I've posted a related request to the Devon forum; 
and am looking at BibDesk.

I'm wondering, though, if it would be more feasible/helpful if the 
Skim Save/Export PDF, PDF bundle, or PDF with Embedded Notes options 
allowed the user to prepend a second copy of the Notes to the 
beginning of the PDF. This prepended Notes page would, in effect, 
announce where in the file Skim notes have been added.
