Sometimes if you load another screen reader like NVDA while jaws is still loaded, it can interfere with the screen reader flag. What I suggest is having an open and close program event in your sound scheme so you know when programs have completely closed themselves. JAWS especially takes a sickenly long time to close sometimes, even after you confirm exit. It can act like it's gone, but the process is still running. So when you load NVDA, the screen reader flag gets turned on, but then the JAWS process closes, turning it back off while NVDA is still loaded. The same could happen when unloading jaws and loading NVDA.

I did know someone who used to constantly have this issue under windows xp though, and we never figured out why. Also, if you ever accidentally load narrator and unload it, that forces the screen reader flag to off as well. I've done this myself several times.

This may seem like a strange suggestion, but try this sometime:

1.  Make sure Skype is not running.

2.  Unload JAWS and make sure there are no other screen readers
running.

3.  Load JAWS.

4.  Load Skype.

The reason for this suggestion is that Ctrl+Shift+S broadcasts the
current state of the screen reader flag, but it can not turn it on.
If you have loaded and unloaded a screen reader while another one is
running, you stand the chance of shutting the screen reader flag off,
which would prevent Ctrl+Shift+S from being of any help.  The sequence
above should guarantee that the screen reader flag is turned on.

If this solves your problem, the next step would be to evaluate your
startup routine to see if something you're doing might be disabling
the screen reader flag regularly.
