Hey Leslie,
I think the best solution with NVDA is that you install SkypeTalking. You
can find it at http://skypetalking.googlecode.com. This software was
primarily intended for NVDA though now it also supports JAWS, Window Eyes
and System Access (including SaToGo).
SkypeTalking hotkeys have, by default, the using of alt+shift+ctrl and
something else, though you can change them trhough SkypeTalking options.
