I think most of the stories have covered the basics
1 - back up
2 - back up often
3 - use automated program backup + your own backup + system
backup
4 - check the logs, check the logs, check the logs, repeat ....
5 - protect your back up media
6 - use quality media
7 - maintain your backup device
8 - regularly test your device for backup and restore (yep, I've
had them back up and NOT restore).
9 - it is best to have multiple devices that can restore the
media (it is useless if that is the only device that can read the
media)
10 - regular test restores (i've found a few sites that had NO
effective backup)
11 - backup and test restore dummy data (yep, found a few system
that 'backed up' and indexed the tape and the system thought the
data was there, but it could not recover anything.
12 - someone will think of something I've forgotten.
