This is not neccesarily the case. You could choose to have no fixed
phone at all (I did this for most of 1999 in Montreal), or, if you use
Optus (granted, pricing will change), your fixed phone can be connected
to the cable service.

Wrong.
In order to place an order for ADSL, you must either have an existing
phone line to put it on or order one at the same time as you order the
ADSL service. The listed ADSL pricing does _not_ include the physical
line itself.
