George that really sounds like you are up for some hardcore postscript
coding. You will probably need to make your own script that reads in the
respective bits and assembles the page yourself in postscript.
The instructions at this page

provide you with a good starter on how to automatically bits into a page.
A messy combination of the faxcover creation scripts, the postscript
parseing stuff and sendfax should be able to get it sorted for you.
See, you can give sendfax more command line options, like notes etc that are
meant for the cover page, instead you replace the notes macro marker with
your own customised page.
That doesnt make much sense until after you have read the above url and
tried to make your own cover page work.
