I'm presently quite happy with dingoblue.
$27.50pm true unlimited account (providing you preselect your
phone for long distance calls with them, otherwise it's $49pm).  

They have a web page listing all the DNS etc settings you 
need and another page with how to configure various linux distros 
(http://k9.dingoblue.com.au).  They're reasonably quick and they 
have this nifty "web chat" thing to "talk" to a customer service 
person, saves waiting on hold.

(Also, hrmmm [clears throat] if anyone should want to join after
that sales spiel, email me and I'll "refer" you and then we both
get $40 account credit each.)
