My personal experience is that Acrobat Reader is the "guaranteed" way to
convert pdfs to postscript.
I use:
$acroread -toPostScript -pairs <in.pdf> <out.ps>
This does a great job of handling all pdfs I have come across (not sure
about v5 Acrobat though).
I thought there was a man page but couldn't find it.
