Basically, it means that one way or another, the OS lost contact with
the disk. This could be a hardware failure of the drive (most likely),
or it could also be a port, controller, driver, overheating, power, and
so on.

As has been already pointed out, the easiest thing to try would be to
swap the disk for a different mSATA and/or a different type to see if
the problem continues.

The timing is a bit suspicious, but there isn't anything on pfSense that
would only kick in once every two weeks. It could be a cumulative number
of writes or similar that is contributing.

Use NanoBSD to reduce the stress on the disk, or if you're on pfSense
2.1, running a full install you can enable the option to use a RAM disk
for /var and /tmp which cuts back on many writes even using a full
install. That doesn't help with most packages though.
