The framebuffer driver adjusts the frequencies of the VGA output when
TV-out is activated, in order to align the VGA out timings to the TV
signal. The TV encoder just encodes the signal without changing the
frequencies.
Thus no change with fbset will have any effect when TV-out is on.
Mark Adams did a very good job to adjust timings in the "tv tables"
inside the framebuffer driver, but did only take PAL into account.
AFAIK, noone ever tried to adjust NTSC setting to proper full-frame
(considering you're using NTSC).
"tv tables" are found in the kernel driver code... You could try to get
old posts from Mark Adams on the directfb-users ML, where he might
explain how he did it. IIRC, this was between september and december 2005.
