Harwin makes connectors like that, they are available from Farnell and 
probably Digi-Key or Mouser. It's easy to cut them down to any required 
number of ways. The crimp tool is expensive but I've found that the cheap 
ones sold for use with Molex KK terminals can be used, with care.
