FWIW If you don't get this sorted out, you may want to try asking on the SURBL discussion list: http://lists.surbl.org/ That said, the behavior you're reporting makes me wonder if your internal/local mail is being processed the same way as your inbound external mail. Can you get other SA rules to trigger when you locally re-send other types of spammy messages, for example one already tagged by other rules?
