It's not a certainty. It only happens with calendars that are being
synced and, in my experience, tends to happen more with events that
are in both a subscribed calendar and a synced calendar as opposed to
two normal calendars. I've spoken to the lead Apple sync engineer
about this, he agrees it's a bug and said they will try to come up
with a better solution (apparently this was done on purpose to prevent
"dumb" phones from duplicating calendar events).
