Usually that would point to a problem with the Apple Sync Services
database and can be remedied by resetting your sync history like so:

* Open iSync (in /Applications)
* Go to preferences
* Click Reset Sync History
* Logout of your Mac OS X account and back in (or relaunch the
Spanning Sync background app manually if you prefer)
* Sync
