Thanks for the suggestion. I reset the shared secret, stopped and started CF and SM
services, but still no dice.

A simple thing to try is to reset the "shared secret" setting in cf
administrator (under Advanced Security). I've found that doing this will
often eliminate mysterious cf/siteminder handshake problems, especially
when I'm pretty sure all the other security settings are correct.
