I've had some thing similar with the linux driver. The problem is that the microcode is not being reset, to do that I removed the USB driver module to power down the modem before restarting the whole lot again.
which seems to work fine. I've since automated this with a cron script to test the line and reset it if not working. I don't know if this is possible with FreeBSD, but hope it helps. Ahmed...
