For parsing the reStructuredText sources, Sphinx uses Docutils_. If you want localized directive and role names, the corresponding translations must be added to Docutils. Patches are welcome, details are given in http://docutils.sourceforge.net/docs/howto/i18n.html (update: specifying and using utf-8 encoding is no longer discouraged).
