If there are combustibles then you would most likely need one at the top
as well here.  
Years ago the Fire and Elevator inspectors fought it out and came up
with a monitored valve (like a butterball va) on each as a "compromise".
You can run them exposed with no box but you will have to drop the valve
down to 4'-0" AFF and loop back up and into machine room/pit/top.

I have a NFPA 13 system with a hydraulic elevator and a machine room in
the basement with a combustible shaft top so in WI we would need to put
one at the top / bottom in the machine room but NO control valves.
If the pipe is exposed at about 7'6" AFF would a box still be required?

Yes, a monitored control valve is required in Michigan.  Location of
valve and box is also important.  Usually if you stand 6'-0" in front of
elevator door and rotate or spin 360 degrees you should be able to
locate it.  If its location is not visible from there they must have
signage pointing you in the right direction.  Of course, it's all on
what the AHJ will approve for location.  
I haven't put a head at the top of the shaft in years but you will need
one in the pit and machine room.  In some places you can eliminate the
head in the pit if it is an electric elevator in a non-com shaft with no
hydraulics.
