Yes, a monitored control valve is required in Michigan. Location of valve and box is also important. Usually if you stand 6'-0" in front of elevator door and rotate or spin 360 degrees you should be able to locate it. If its location is not visible from there they must have signage pointing you in the right direction. Of course, it's all on what the AHJ will approve for location. I haven't put a head at the top of the shaft in years but you will need one in the pit and machine room. In some places you can eliminate the head in the pit if it is an electric elevator in a non-com shaft with no hydraulics.
