As I went through some classes a couple months ago (OSHA & MSHA).  No specific 
requirement for the ladder at that height (it kicks in at 15-20' or so for 
"fixed" ladders), the fall protection requirement kick in based on what is 
around you (leading edge, machinery, open containers, etc.).  .  Working 
platforms are also worded so that the requirements are 6' and one "could" argue 
the platform/ladder angle.  MSHA is a different animal all together.

I am willing to be wrong, but in the last 18 years I have never heard that feet 
can be more than 6' off the ground on a ladder without being tied off to the 
structure.

If I am wrong, I would love to have the documentation to fight this issue when 
necessary.
