Here's what I found (NFPA 13 2013):
6.1.1.5 - isn't required to be listed, only approved.
6.7.1.3 - not required to be a listed indicating valve.
6.7.1.2 - not required to be 'slow-close'.
6.7.3 - must be 'approved'

So, being mostly design only, I don't have direct access to net pricing.
But if the above is correct, and if a 4" non-listed, non-slow-close valve
cost less than the typical listed 4" butterfly valve, I can't see why a
contractor can't consider using something similar to Nibco GD-4765
(http://www.nibco.com/Valves/Butterfly-Valves/Ductile-Iron-Butterfly-Valves/
GD-4765-Butterfly-Valve-Ductile-Iron-Grooved-300-PSI/).  Anybody see it
differently?  Any contractors want to price out the two different types of
valves and post the info?

No. 

The building code requires, "All valves controlling the water supply ...
" for automatic sprinkler systems, pumps, tanks, etc. be supervised.   A
test valve is not a fire protection water supply control valve.

Without going back through the rest of the mile long thread.......

IF there is a control valve in the normally closed position for Full Forward
Flow Testing,  is it REQUIRED to be monitored?
