Maybe it happens when it's not one character but a "compound" of two
characters? What I mean is, you can use the character á, but you can
also "build" it with an a and an accent, like this: а́

á
 U+00E1 : LATIN SMALL LETTER A WITH ACUTE

а́
U+0430 : CYRILLIC SMALL LETTER A

U+0301 : COMBINING ACUTE ACCENT {stress mark; Greek oxia,
tonos}

I've seen this with the letter ö quite often in news outlets like dpa.
Depending on the font used, the dots may not end up on top of the o.
