I think this happens if the following occurs:
- An album is added in a scan. - After this scan, some of the album's files get a new "modified" time tag (for example, by pressing "save" in mp3tag) - A second scan looks "for new and changed media files" and finds that only some files changed in that album. - A second album is created with these changed files. (This should not happen.) And yes, "Clear library and rescan everything" helps here.
