First step will be to force the complete rescan.  I've been in a slow
process of reripping CD's to FLAC and partaking in a similar process to
replace files in the library.  LMS struggles a bit with replacing files
in this way as there are almost always slight differences in the file
information that confuses LMS and leaves files that no longer exist in
the database.

Aside from a rescan, when your files are brought in with tags outside
your control (happens for me with the initial rip relying on internet
data to fill tags or when buying tracks that are already tagged), you
have to look at almost every tag that is there for consistency. Here are
just some examples where I've had files show up as different albums (or
split among artists):

1. Mixed capitalization in the album title (or even in album artists): 
e.g. Dark Side of the Moon vs.  Dark Side Of The Moon.  
2. Inconsistent discnumber (and its variations) tags:  e.g. 1/1 vs 1 
(or some filled and some blank, or some 1 and some 2, etc)
3. Compilation tags (similar to disc number):  I've had albums where
there are guest artists be interpretted as compilations.  I usually
force a compilation=0 tag.  Some are against doing that, but it is
easier for me with MP3Tag to just force it to 0 when I know albums
aren't compilations than deal with the random misinterpretations
4. Inconsistent sort tags (this rarely happens, but it can impact things
just like any of the above).

With MP3Tag, I find it works well to select all the tracks in an album. 
For certain tags that you know should be consistent across an album,
they should all show the same value.  If they show nothing or that the
results vary, then you can now change them to be consistent.  

Occasionally after going through the steps of cleaning up the tags I
have to clear and rescan the database.
