I used Foobar 2000 1.6.2 and opusenc.exe based on Libopus 1.3.1 in
opustools 0.2.
One instance, I left the default .opus ending, the other I changed to
.ogg. Same source FLAC file.
Didn't add any funny tags yet, such as replaygain or albumart. Should be
as vanilla as possible.

Where do we go from here? Can I do anything else to help troubleshoot?
Out of curiosity: How did you get that detail on the scan? All of my
scanner logs are set to "debug", but there still isn't anything telling
in them...
