So I was running squeezelite with my default (hardware) MAC address.  I
gave my player a dummy MAC in the 00:04:20 range and NOW everything
works again.

As Pippin said: don't do this. It's asking for trouble. Not only for you, but for some fellow SB user, too.

And as I said before: what you're seeing can happen if a player's mac already is registered, and/or if it's using a UUID which does already exist. Most software players don't use a UUID. Therefore duplicates are very likely.

Is this something new that mysb.com is rejecting MAC addresses outside
this range?

No.

I imagine that there would be a lot of people running
non-logitech hardware that would run into problems like this.

The longer, the more. In particular the VB case you mentioned, where many players are sharing the same fake MAC address.
