This is what i use and it works fine, if i want to deny avi file downloads i add \.avi \.AVI
acl dnddlwds urlpath_regex \.asx \.ASX \.ram \.RAM \.exe \.EXE
http_access deny dnddlwds

I saw this some time ago on a squid 2.4 Stable 7. You can use the acl and it also works, bit when you change something and do a squid -k reconfigure you get what you say: all access is denied. Restart the whole squid instead and all is fine >
