use "-i"  then it's not case sensitive.
And you should use "$" , for example :   \.asx$ \.ram$

I saw this some time ago on a squid 2.4 Stable 7.
You can use the acl and it also works, bit when you change something and do
a squid -k reconfigure you get what you say: all access is denied. Restart
the whole squid instead and all is fine
