Try bumping memory_limit in php.ini up also. It's 8M by default which you'd think would be enough but when the attachment is encoded and Squirrelmail is keeping track of other things 8M might not be enough. I'd say try bumping this and also upload_max_filesize to something like 64M. See if that fixes the problem. Then lower until the problem reappears. You'll need to decide how large an attachment you will allow. You should also upgrade from 1.4.1 to 1.4.3a as it contains a number of bug fixes and security patches (although none related to attachment issues that I recall). Still it's a good idea.
