I am fine to create acc/missed_calls with the extra columns that are specified
in `db_extra` modparam of the default kamailio.cfg and remove the sql
statements. Eventually the kamctl/kamcli can get the subcommand to drop the
extra-columns, as a way to reset to mandatory columns.
Just removing the comments in the default kamailio.cfg without having the
columns created wouldn't be ok, imo, because default config won't work out of
the box, without any clear steps inside its docs.
