Greetings, Not that I know necessarily what's going on, but I've had adventures with PCI cards on the StarMax also. After upgradinging to OS 9.1, the PCI card for USB port didn't appear nor did anything pugged into it (by Apple System Profiler). The card required older Apple USB driver and other extensions (e.g. - v1.3.5 USB Support, USB Software locater, USB Device extension, SerialShimLib, HID Library ). I've gone through the thread here looking to see if you've mentioned the USB card manufacturer and didn't see it. That might help to figure out which version of USB extensions to use... If you go to your extensions manager, you should be able to see what versions you currently have in the StarMax.
