I can't speak for cards, as I have not tested that.
We don't use smart cards here.
The problem is when the users does a utdetach, or is disconnected via
utdetach the x11vnc stops responding.
The developer of x11vnc is aware of the problem but needs access to a
system to test and develop on. I am actually trying to get a public
system up and running for him to try it out on.
