with the other failures, i'm inclined to think it is dying slowly. that type of slow progressive death can be cause by a lack of cooling when the fan fails (although you'd probably get allot of crashes before permanent damage) or more likely it took a surge. i've seen equipment that took a surge before, i wasted my time replacing parts twice in as many weeks before i realized parts were going to keep on failing. a smaller surge could do the same thing on a longer time scale, surges tend to damage many components enough that they fail later.
