Heres the extra info you asked for:

Has this been your setup for a while and this problem surfaced recently 
or were there changes ?

Over the last few months I have upgraded my system with the following 
items:
Sonnet G4/800 (previously Sonnet G3/333)
OS 9.1
ATI RAdeon 7000
The scanner worked prior to these upgrades.
 
Is the scanner a recent acquisition ?
NO, have had it for several years.

Something keeps pointing to a problem with the driver.
Are you at least making it to the scanner control window when you 
"acquire" the scanner in Photoshop ?
No never get this far. This is where the error message appears.
