Mine wouldn't boot with 2 video cards in it either, so I just used the ATI 
card, and it does continue to work during/after being flashed.   That's 
because DOS doesn't use the card's on-board ROM.

Yeah I enabled "Quartz Extreme" and didn't really see much difference.   I'm 
running a Sonnett G4/450 CPU.

Yes I'm running 10.2.4.   Under OS 9 I didn't do anything.   I downloaded 
the ATI drivers but didn't bother to install them since OS 9 works OK for 
what I use it for --- installing and booting OS 10!
