David,

You get a picture in both Mac OS 9 and Mac OS X, so the flash seems to be
successful.
The rest: Driver or configuration problems, I think.

First: The ATI requires slot A or B, even the original Mac version does not
work on the PCI bridge, at least not mine. Try it in Slot A, if it doesn't
work, post back.
