Could it matter what slot the flash is done in on the Mac side, and/or is there any hope of booting from a DOS floppy on the PC side to attempt completion there if it apparently prevents all booting now?
Looking back, the only option for the initial flash would have been to run FLASHRM3 (renamed) instead of the normal one, but according to the directions that should only have been necessary if an "ASIC" or "reg map" error was encountered.
That seems to limit the blame either on my buying the wrong card or using the wrong ATi ROM updater (the only one available I could find at ATi), unless I should have moved it into another slot in the Mac. That seems a longshot since nowhere have I seen that being an issue in order for the ROM update to run. I tried running it several times and got the same result.

When flashing on the Mac including the Umax machines it shouldn't matter which slot the card is in. However after flashing a Umax S900/J700 requires that it be in the A or B slot (top in the S900) in order to use the card.
It sounds like the flashing went ok in the PC and the issues is with the Mac flashing but not sure.
For the record it is best to flash on the PC when booted from a dos boot floppy and not the hard drive. One advantage of this is dos doesn't use the drivers so the flashed card will still boot into dos regardless if it's PC or Mac flashed.
You will know if the Mac Rom is flashing correctly as it will show a progress bar in a small window. If you don't see this progress bar it isn't running correctly. It took me several times before it ran for me. Apple system profiler will then show the rom file number and that it is an Aty card. There should be no reason to turn extensions off in fact this may be the problem.
Another possible problem the Mac Rom flasher currently at ATI may or may not work on the Sapphire card. ATI pulled the flasher for a week or so after taking legal action against web sites which were posting the file. It was speculated they may have made changes so it would only work in the ATI cards. It's not known if this is true or not. I do know that i have a copy of the original file which I could send you. It has the same name but is older.
You can get this to work but may take some work. The card can be flashed as many times as is needed to get it to work. So one bad flash isn't going to stop you from doing it again.
