Grasping for straws here, but which ROM file did you use. At one point there were two ROM files, a partial file, and a full 128K (?) file. If you use the full ROM file, then you don't need to flash anything on the Mac side. The PC will load the full 208 ROM into the card. This was the method that I used, IIRC it was the most successful with the Sapphire 32MB cards.
As for the PC not wanting to boot up. I had the problem on my Dell (on board video, and also has an ATI Mach64 card). The way I got around it, (I know this will sound wierd) was by disabling the hard drives in the BIOS and then unplugging them from the machine. I then used the ATI Mach 64 card to see what I was doing while the machine booted from a floppy disk and flashed the Radeon. It worked great! Maybe the PC you are using has an issue like this.
