CORRECTION: Unless all three are UNchecked for all your accounts,
SM will check, and for those accounts where the third option is
checked, it will download messages. It's not enough to clear the
boxes for one account.
And why is this NOT on a per account basis?
It is, as I said. You can have different settings for each account.
