Is it possible that the recipient has blocked the automatic opening of
attachments?  Even inline images in HTML-formatted E-mail messages
actually traverse the Internet as attachments, separated from the
message as it is sent and (if allowed by the recipient) recombined with
the message upon receipt.  However, that recombination will not occur if
the recipient is blocking the automatic opening of attachments.

Why block images?  Even images have been found to contain malware.  Just
go to
<http://search.us-cert.gov/search?utf8=%E2%9C%93&input-form=advanced&affiliate=us-cert&query-or=JPEG+GIF+PNG+BMP&per-page=10&filter=off&x=31&y=9>.
 That lengthy URI is a search query of the Web site of the US-CERT, an
office within the U.S. Department of Homeland Security.  The search will
show a large number of vulnerability alerts and bulletins relating to
malware in JPEG, GIF, PNG, and BMP files.
